We are building a qualified talent network for a focused mix of part-time grant-funded employment and project-based professional services supporting the Community Wellness and Navigation Program.
Coordinate outreach, enrollment and program delivery; guide participants through available resources; conduct follow-up; maintain program records; and assemble routine outcome information for reporting.
Facilitate accessible learning conversations and, where relevant, provide peer-informed recovery and resource-navigation support. Engagements are scheduled around specific sessions, participant needs, or funded projects.
Review outcome systems, analyze aggregate program data, document results, and support accurate funder and board reporting through defined professional-service engagements.
